Rehab Coordinator, PTA - Peshtigo, WI

Job Summary: The primary purpose is to coordinate therapy services in terms of RUG’s management, caseload development, program development, therapy staffing, participation at key therapy and interdisciplinary meetings, be a treating therapy assistant 50% of the time, work with other sister center’s in staffing and education, coordination of services.

Essential Job Functions:

1)      Carry out the physical therapist’s plan of care, contribute to the ongoing assessments, update the physical therapist with any changes to the patient.

2)      Follow State Practice Acts/Scope of Practice as a PTA.

3)      Guide the therapy department in abiding by State Practice and Medicare Guidelines.

4)      Complete screen process with PT input.

5)      RUG’s management.

6)      Chronic Disease management.

7)      Facilitate staff toward caseload development to meet the needs of resident’s of the facility.

8)      Receive and carryout direction from the Director of Rehab (DOR) and Corporate Director of Therapy Services (CDTS).

9)      Work with the DOR on staffing both Peshtigo center’s appropriately.

10)   Contribute to recruitment, hiring, orientation, recruitment, hiring, education, discipline with DOR, Administrator and CDTS oversight and direction.

11)   Coordinate interdisciplinary staff to meet the needs of the residents and needs of the facility.

12)   Work with the Administrator on center needs.

13)   Assure the safety and privacy of the department and the residents.

14)   Manage and order supplies, equipment as appropriate within the budgetary guidelines.

15)   All other duties as assigned.

 

Requirements: Graduate of an accredited PTA school, valid license with no reprimands, minimum 5 years experience in a SNF and coordinator experience in a SNF. Other management degrees preferred but not required. Continuing education demonstrating a well rounded list of expertise, management a plus.
